In this Clinical Short, we address a common dilemma faced by practitioners when managing larger root canals. We recently received a great question from a member of the Contemporary Endodontics community who asked:

"Firstly, I just wanted to say that the Contemporary Endodontics website is fantastic, I’ve found it incredibly useful. I had a question regarding apexification. 

Traditionally, I’ve tended to consider apexification when the apical diameter is around a size 50–55. However, with the increased use of bioceramic sealers, I was wondering whether you feel this threshold has changed and whether larger apical diameters can be managed predictably with conventional obturation techniques. 

I’d be interested to hear your thoughts and what criteria you use when deciding between apexification and obturation with bioceramic sealer."

In the video we explore the properties of bioceramic sealers, the technical challenges of "washout" and flow control, and whether a single-cone technique can truly replace traditional apexification for size 50–60 apices.
